I authorize the references listed in this application and any prior employer, educational institution, or any other persons or organizations to give SecurAlarm Systems, Inc. (the "Company") any and all information and opinions concerning my previous employment/education accomplishments, disciplinary information or any other pertinent information they may have, personal or otherwise, and release all parties from all liability for any damage that may result from furnishing same to you. I hereby waive written notice that employment information is being provided by any person or organization. I authorize the Company to secure criminal conviction history from law enforcement agencies.

If I am hired, in consideration for my employment, I agree to abide by the rules and policies of this Company, including any changes made from time to time, and agree that my employment and compensation can be terminated at will, at the option of either the Company or myself. I understand that no one employed by the Company has any authority to limit in any way the Company's right to terminate employment at will, or to offer employment other than on an at-will basis.

I agree that any action or suit arising out of my employment or termination of employment with the Company, including but not limited to claims arising under State or Federal civil right statutes, must be brought within 180 days of the event giving rise to the claim or be forever barred. I understand that this means that even if the law would give me the right to wait a longer time to make a claim, I am giving up that right. If this 180 day period is ever found by a court to be too short to allow its enforcement, I agree that the 180 day period should be extended day by day to the extent necessary to allow its enforcement.

If I require an accommodation to perform the job, I must notify the Human Resources Department in writing of that need within 182 days after I know or reasonably should have known that an accommodation was needed under the Michigan Handicappers' Civil Rights Act. Failure to do so will bar me from alleging that the Company has not accommodated me as required by that law.
